Why does an item not appear in QuickBooks Online after it was submitted in Dext? (Dext-ApprovalMax-QuickBooks Online integration)

The most common reason for this is that the "Default Tax for Costs" in Dext has not been specified:

If this setting is missing, QuickBooks Online will simply not accept items without tax.
